Documents Needed for Immunizations
Photo ID: Required for adults.
Immunization record: Bring a copy of your own or your child’s immunization record. Parents must bring a current copy of each child’s immunization record.
Consent form/authorization note: If a legal guardian is not present, a signed note from the legal guardian must be provided, which includes:
The child’s name and date of birth
The name of the adult (18 or older) bringing the child
The name and signature of the parent or legal guardian giving permission
This ensures all necessary documents are in place for the immunization process.

Frequently Asked Questions
“What if I can’t pay for STD testing?”
Some fees can be waived. Speak with the receptionist at the Public Health clinic when making your appointment. No one will be turned away due to an inability to pay.
“How old do I have to be to get tested for an STD?”
If you are 13 or older and sexually active, the Public Health clinic can test you for gonorrhea, chlamydia, syphilis, and HIV. Parental permission is not required if you are ages 13-18.
“How long does it take to get my STD test results?”
Typically, STD test results take 7-10 days. The Public Health Department will notify you of your test results and provide any necessary follow-up care or treatment based on the outcome.
“How do I know if I need emergency STD care?”
Seek emergency care if you experience:
Severe genital or rectal bleeding
Severe abdominal or testicular pain
A fever
A foreign object stuck in your vagina or rectum
The Public Health clinic can guide you to emergency care if needed.

You remain eligible for our Refugee Health Services for up to one year after arriving in the U.S. This timeframe allows us to provide crucial health support during your initial settlement period.
Free vaccines are available to refugees who have been in the U.S. for less than 12 months, as part of our commitment to promoting public health within the refugee community.

Womens Health
Services are available to women who are residents of Potter and Randall Counties.
Appointments are Required
Clinic hours are Monday through Thursday, 7:30 am – 6:00 pm

Options are available for a sliding scale fee — based on qualifications
Age requirements for the Women’s Health Clinic vary by service.
Cervical Cancer Screening: Available for individuals 21 and older.
Mammogram referrals: Available for those 40 and older, or younger if you have a family history of breast cancer or are experiencing symptoms.
Contraception Options: Available for ages 13 and older. Please note: Parental consent is required for ages 13-17.
Resource Navigation: No age limitation.
Pregnancy Testing: Available for all ages.
STI/STD Testing & Treatment: Availability for adults; ages 13 to 17 is limited to HIV, gonorrhea, chlamydia, and syphilis.
Medicaid navigation: No age requirement, but you must be pregnant.

The Local Outreach to Suicide Survivors (LOSS) Team offers a range of resources and services to help Survivors of Suicide through the grieving process, at no cost.
LOSS Program Highlights
Immediate or Follow-Up Support
Compassionate care and guidance are provided on-site or shortly after a loss.
Resource Binders
Comprehensive resources, including counseling services, grief support groups, and practical guidance for navigating the aftermath of a suicide.
Follow-Up Care
Ongoing support and resources to help survivors in their grief journey
Suicide Grievers Support Group
The Suicide Grievers Support Group provides a safe and welcoming space for individuals who have lost someone to suicide. This peer-led group meets monthly at no cost, offering an opportunity to connect, share experiences, and find support in a compassionate environment.
